INTER-PROFESSIONAL PRIMARY HEALTHCARE

Physicians – treat acute and chronic illnesses, diagnose and provide preventive care and health education for males and females and from newborn to older adults.

Nurse Practitioners – Nurse Practitioner are Independent and Qualified healthcare providers and provide a full range of health services including diagnosis, treatment, consultation, specialist referrals, conducting physical check-ups, administering and analyzing tests etc… Nurse Practitioners play an important role in helping the community maintain a healthy and active lifestyle and to understand and/or treat illnesses.

Registered Nurse – The Registered Nurse is the first point of contact for all TAIBU clients. The Registered Nurse also does well baby and child checkups, blood work, preventative screening, including gynecological exam, discussion about mammography and fecal occult blood test. He/She works along with the Physicians and Nurse Practitioners in our Hypertension Management Program and Sickle Cell initiative. OUR PROGRAMS & SERVICES

Diabetes is a serious chronic condition. Black people are two to three times at greater risk of developing Type 2 Diabetes.

TAIBU has a team of health professionals that make up the Diabetes Education Program (DEP). The mission of the DEP is to improve the quality of life for people affected by Type 2 Diabetes by providing a culturally and linguistically appropriate service and high standard of diabetes care and education that promotes self management.

We offer a comprehensive Diabetes Education Program to provide practical information on diabetes through our multi-disciplinary team including a Registered Nurse Educator, Registered Dietitian Educator, Chiropodist (Food Care Specialist) and a Social Worker.

TAIBU’s goals for this program are:

  • To reach as many community members who are at risk of developing Type 2 Diabetes, to increase their knowledge of the seriousness of diabetes, it s risk and the effective strategies to prevent Diabetes and its complications.
  • To provide personal care that promotes healthy living with a highly supportive network to help maintain positive changes in lifestyles.
  • To build effective partnerships with other agencies as well as network and share best practice models as they relate to racialized and marginalized communities.
  • To refer patients, families, health care workers, students and community groups to appropriate diabetes resources.

Our services address a wide variety of issues spanning from trauma (sexual and physical), depression, mental health, anxiety, low self esteem and/or societal stressors relating to the Social Determinants of Health (racism, poverty, housing, employment, family issues, etc.). The Social Worker/Therapist works from an anti-oppressive framework which encompasses both teaching and learning from clients about the different forms of oppression that occur simultaneously in their lives on a day-to-day basis. Our therapeutic model involves an integrative process involving Cognitive Behavioural Approach, Narrative Therapy, and therapy from a Recovery Model of Care.

Through therapy clients are empowered:

  • To play an active role in the development of their recovery model of care.
  • To engage in a process of personal empowerment which leads to self-awareness and clarity
  • To engage in a highly effective way to move forward when feeling stuck and to move past emotional pain and discomfort.
The practice of Chiropody is the assessment, treatment and prevention of diseases or disorders of the foot by therapeutic, surgical, orthotic and palliative means. An essential member of the integrated professional healthcare team, the Chiropodist is skilled in assessing the needs of our clients and of managing both chronic and acute conditions affecting foot and lower limb function.

The role of the Chiropodist is to maintain and improve patient mobility, to alleviate foot pain, provide health education and thereby improving one’s quality of life. The diverse range of services and conditions treated include: custom-made orthotics; custom-made orthopaedic footwear; general foot care-corns, calluses, nails, heel, arch, forefoot, and knee pain; flat feet care; wart removal; ingrown toenails; athlete’s foot and fungal nails; diabetic foot care including ulcers; arthritic foot care; sports injuries; and children’s foot care.

A Registered Dietitian is a health care professional who is trained to provide advice on diet, food and nutrition.

TAIBU offers nutrition counseling for individuals and groups. Get expert nutrition advice from TAIBU’s registered dietitians to help you manage a wide range of diet related diseases such as hypertension, high cholesterol, obesity and diabetes. We also offer advice on prenatal nutrition and feeding in the early years (picky eaters, food allergy and failure to thrive).

TAIBU CHC was one of the seven successful applicants in securing funding for the new Low Back Pain Project from the Ministry of Health and Long Term Care.

‘Back 2 Life’ is a two year low back pain management program with the aim of providing relief from pain and suffering through assessment and treatment, life style changes, education and support, physical activity and massage therapy. The program incorporates the services of a Chiropractor and kinesiologist.

‘Back 2 Life is a partnership with Morgan Chiropractic and Wellness Centre and the Centennial College. The partnership with Centennial College is for the aspect of the complimentary massage therapy.

TAIBU has a partnership with Ontario Telemedicine Network (ONT) to provide health care services through secure and convenient video teleconferencing system. This system helps our clients by bridging the distance of time and geography so that we can bring more clients the care they need, where and when they need it.

If the system is available at your specialist’s/provider’s office (e.g. Psychiatrist, Cardiologist, Dermatologist), you can book appointment through our Telemedicine Nurse Coordinator. You will attend your appointment at TAIBU’s facility and you will be able to discuss your health concerns and follow up consultation with your specialist/provider in a secure and confidential space.
