21st Century Chiropractic – Where are we Going?

The World Federation of Chiropractic has recently proposed a 21st century model of chiropractic care known as “EPIC”: Evidence-based, People-centered, Inter-professional and Collaborative. Dr. Schneider believes that the profession should not be content with the goal of merely surviving for another 100 years. Instead, he suggests that the profession should set its sights higher, with the goal of thriving in the 21st century. He will discuss the “EPIC” model, as well as the many challenges and opportunities that the chiropractic profession needs to address as it prepares to thrive as a mature and mainstream healthcare profession.
Webinar participants will have an opportunity to submit questions during the webinar.

A Review of the Evidence for Rehabilitative Exercise: Which Exercise for which Patient?

The rehabilitative exercise literature is full of articles that provide evidence for the clinical effectiveness of different types of rehabilitative exercise for patients with low back and neck pain. There are also a number of competing theories about which is the “best exercise” for patients with spine related disorders. Dr. Schneider will review the evidence for several of the most popular rehabilitative exercise approaches, including directional preference (McKenzie), spinal stabilization/neutral spine training, sensorimotor control exercises, and general strength and conditioning exercises. He will also present an overview of the movement control approach used within the Treatment Based Classification system to determine which type of exercise is best for which type of patient.

Clinical Reasoning and Rehabilitation: Linking Physical Examination to Exercise Prescription

REHAB BOOTH

Dr. Cambridge's presentation will examine clinical rationale and critical thinking in the prescription and execution of rehabilitation exercise. This lecture will utilize a clinical case study to demonstrate the importance of matching a specific assessment to a specific treatment approach. Utilizing an evidence-based, patient-centered approach we will examine the foundational scientific evidence and demonstrate how that can be used to address specific clinical concerns. Included in this presentation are aspects from Dr. Cambridge's thesis which provide an overarching theory and rationale for clinical decision making and the implementation of rehabilitation strategies.
To meet with Dr. Edward Cambridge to chat and ask any questions you may have, please drop by the CMCC ConED Rehabilitation booth on Thursday June 3, 3:45 to 4:15 ET.

Rehabilitation Protocols for Cervical and Thoracic Spine

REHAB BOOTH

The purpose of this presentation is to discuss rehabilitation protocols in patients with cervical and/or thoracic problems. We will be discussing this within the greater scheme of patient care as a whole. As such, the process starts with diagnosis. The challenge of diagnosis in spine pain patients is that we have to consider the disparate biological, psychological and social factors that can potentially contribute. What is more, there are no clear objective tests that clearly identify the diagnosis in the majority of patients. So the diagnosis is usually based on history and exam. Clinical Reasoning in Spine Pain® help us organize our clinical reasoning process in establishing a diagnosis and formulating a management strategy that is most likely helpful in each patient. Our management strategy may entail methods targeting sources of nociception (manipulation, end range loading, neurodynamics, etc.) and may entail methods targeting perpetuating factors, that usually includes some form of formal rehabilitation (motor control exercise, oculomotor exercise, graded exposure, etc.). We will be discussing specific rehabilitative approaches that are designed to serve a particular purpose in patients with cervical and/or thoracic problems, in light of decision-making and clinical application.
To meet with Dr. Donald Murphy to chat and ask any questions you may have, please drop by the CMCC ConED Rehabilitation booth on Thursday June 3, 4:15 to 4:45 ET.

Mechanotransduction at the Molecular Level

We all know that chiropractic care works very well for the most common complaints that come into our clinics. What is unknown is how chiropractic care achieves its results. One of the major thrusts of research at CMCC is to explore the underlying physiology that translates adjustment and soft tissue therapy into clinical benefits. This is one part of the field of 'mechanotransduction'. In this presentation, we will summarize what is known about how individual cells sense the forces that are being applied to them, and how they respond differently depending upon the nature of the force. This work helps us to understand the subtleties of manual therapies and perhaps explains how a small change in technique could have large implications for our patients.
To ask Dr. Budgell any questions, please attend the Research Panel 1 Q&A webinar on Thursday June 3, 4:30 to 5:30 PM ET.

Dr. Brian Budgell

DC, PhD

Dr. Budgell is a neurophysiologist with an interest in somatovisceral interactions and the health of the spinal cord. The bulk of his papers pertain to the effects of somatic (muscle, bone, joint) stimulation on visceral function, especially cardiovascular function. He has contributed chapters to several clinical texts and was a field editor for Springer’s Encyclopedia of Neuroscience. Through efforts to assist colleagues in publishing their own research, he also developed an interest in biomedical and health linguistics, and has contributed to several corpus linguistics studies of specific health disciplines including nursing, midwifery and public health.

Triggering Cellular Responses

"This presentation focuses on some of the results of the study we’ve done on the effects of a limited series of spinal manipulative interventions (SMT) on the production of inflammatory mediators in patients presenting with low back pain. Importantly, the production of such mediators (potential biomarkers) has been determined in whole blood cultures prepared from blood samples obtained pre- and post-SMT.
The data we’ve obtained indicates differences in the inflammatory profiles of acute vs chronic LBP patients. Furthermore, there are SMT-associated changes in the production of specific cytokines, which vary in the acute and chronic cohorts. I will present data in the context of both within group and between group differences.
To ask Dr. Injeyan any questions, please attend the Research Panel 1 Q&A webinar on Thursday June 3, 4:30 to 5:30 PM ET.

Dr. Stephen Injeyan

DC, MSc, PhD

Dr. Stephen Injeyan

DC, MSc, PhD

Dr. Injeyan received a BSc in Biology and an MSc in Tropical Health from the American University of Beirut, Lebanon and completed his PhD studies at the Institute of Parasitology in at McGill University.
After time teaching and pursuing research at the University of Manitoba and the University of Toronto, he became interested in chiropractic and received his education at CMCC, graduating in 1984. He has been in private practice since graduation, as well as serving as a faculty member in various academic and administrative capacities. He currently holds the title of professor with duties and activities in research and clinical education.
Dr. Injeyan’s research interests have included host–parasite interactions and cellular- morphological responses to external stimuli using various biological models. As well, he has published on topics relevant to public health and to chiropractic education. In the past several years his primary focus of research has been on the exploration of the effects of spinal manipulation on selected parameters of the immune response. He has numerous (48) publications in peer reviewed journals. Dr. Injeyan currently leads the effort in understanding the effects of a manipulative intervention on inflammatory changes in low back patients.

The Adjustment: Utilization, Effectiveness and Role in Clinical Care

The spinal adjustment has defined the profession since its inception. It is one of the most common interventions used by chiropractors in care of their patients. Research suggests that the adjustment or manipulation and soft tissue therapy is provided in up to 90% of chiropractic visits. However, the frequency and type of manual adjustment will vary by patient condition and age. For example, in older adults, soft tissue is most commonly used, followed by manipulation, mobilization and activator compared to patients less than 65 years of age. Systematic reviews and clinical guidelines support the use of manipulation, especially when included in a multimodal care plan, in the management of many musculoskeletal problems. Similar support for non-musculoskeletal problems is lacking. However, the frequency of non-musculoskeletal problems presenting as primary complaints in chiropractic offices is low, with the majority of problems seen involving the low back, neck and joint sprains and strains. This presentation highlights the use and effectiveness of the adjustment in the care of patients. It also points to the risk of having it define the profession rather than by its full scope of practice.
To ask Dr. Mior any questions, please attend the Research Panel 1 Q&A webinar on Thursday June 3, 4:30 to 5:30 PM ET.

Dr. Silvano Mior

DC, PhD

Dr. Silvano Mior graduated from CMCC in 1980 and received his Fellowship with the College of Chiropractic Sciences in 1984. In 2010, he completed a PhD from the Department of Health Policy Management and Evaluation, Faculty of Medicine, University of Toronto.
Dr. Mior has served as Dean of Graduate Studies and Research, and as Vice-President Research and External Relations. He has been an Associate Editor for the Journal of the Canadian Chiropractor Association, the Journal of Neuromusculoskeletal System, The Journal of Sports Chiropractic and Rehabilitation and other professional journals.
He has authored numerous articles and book chapters in areas related to clinical chiropractic practice and outcomes and has participated in international and national committees establishing guidelines for quality assurance and standards of chiropractic practice, as well as setting research agendas. Currently a professor in the Division of Research, Dr. Mior also runs a private practice in Markham.

Bringing Force-sensing Technology into Clinical Practice

The use of force-sensing technology allows us to better understand spinal manipulation forces – their characteristics and action on the body. This presentation includes a brief overview of 3 studies looking at the forces acting on the patient’s body during a spinal manipulation: the forces applied by the clinician measured at the clinician-patient interface and the ones measured at the patient-table interface. Are they the same? Are they different? What changes them? And how can we use them? Findings from these studies are also contextualized in terms of injury and safety.
To ask Dr. Funabashi any questions, please attend the Research Panel 2 Q&A webinar on Friday June 4, 4:30 to 5:30 PM ET.

Dr. Martha Funabashi

BSc, MSc, PhD

Dr. Funabashi is a PhD currently working as a clinical research scientist at CMCC. She is also a CARL fellow and the lead study coordinator of SafetyNET - an international and multidisciplinary research team to support patient safety among spinal manipulative therapy (SMT) providers. Martha has a Master’s Degree in Neuroscience from the University of Sao Paulo - Brazil and completed her PhD in Rehabilitation Sciences at the University of Alberta under the supervision of Dr. Greg Kawchuk. Martha’s research interests and passion are on the SMT’s biomechanics and its safety aspects. Martha has recently won the New Investigator Award at the World Federation of Chiropractic Conference 2017. She has also published important papers in these areas and works in collaboration with emerging and well-known researchers around the world.

Impact of Force on Biochemical Markers and Pain

A mechanism-based approach to diagnosis and management of pain has been widely endorsed by pain specialists. Inflammatory biomarkers play a role in nociceptive, neuropathic, and nociplastic pain. They hold the potential to be used as a diagnosis and response to treatment biomarkers. This presentation summarizes findings of spinal manipulation acting on biomarkers of oxidative stress and inflammation at the spinal cord and systemically. It also summarizes recent study findings looking at distinct force magnitudes driving different responses on blood biomarkers of oxidative stress and inflammation. Ultimately, this presentation raises the question of whether blood biomarkers may be used as a response to spinal manipulation treatment biomarker in conditions where inflammation plays a role.
To ask Dr. Duarte any questions, please attend the Research Panel 2 Q&A webinar on Friday June 4, 4:30 to 5:30 PM ET.

Dr. Felipe Duarte

DC, MSc, PhD

Dr. Felipe Duarte

DC, MSc, PhD

Dr. Felipe Duarte is a postdoctoral fellow in CMCC’s Life Sciences Lab. He has a Bachelor of Science degree in Chiropractic from Feevale University, Brazil and a Master’s Degree in Human Physiology from the Federal University of Rio Grande do Sul, Brazil. He completed his PhD from the Neuroscience Department at the University of Guelph, supervised by Dr. John Srbely and is the first Brazilian chiropractor to receive a scholarship from the Brazilian Government to pursue a PhD internationally.
Dr. Duarte’s research interest is in the cross-talk between the nervous and immune system and the role of their maladaptation in chronic MSK diseases. Particularly, he has recently investigated the function of neurogenic inflammation as the underlying mechanism of osteoarthritis and myofascial pain syndrome. As a mechanistic chiropractor researcher, he is also keen to understanding the effects of chiropractic care and SMT on the neurophysiological and neuroimmune system throughout preclinical, experimental and clinical investigation.

Force Patterns and Delivery Effects

When planning to use spinal manipulation therapy, clinicians must make several decisions related to their intended effects for the treatment. For example, what technique to use, contact location(s), patient posture, force magnitude and force direction to name a few. Factors that are considered in these decisions have been referred to as “control parameters” that are related to the patient, provider and/or procedure. This presentation covers a study that determined if modifying control parameters (patient posture and technique), representing clinician intent, was reflected in biomechanical conditions experienced by the patient. Findings from this study are then contextualized by considering approaches for using spinal manipulation therapy in patients with intervertebral disc herniations.
To ask Dr. Howarth any questions, please attend the Research Panel 2 Q&A webinar, on Friday June 4, 4:30 to 5:30 PM ET.

Dr. Samuel Howarth

MSc, PhD

Dr. Samuel Howarth is CMCC’s Director of Human Performance Research and the McMorland Family Research Chair in Mechanobiology. His current research focuses on the quantitative assessment of movement and the influence of different constraints on movement patterns utilised by healthy individuals and those with different musculoskeletal conditions. This work is ultimately aimed at identifying attributes of movements that could be used as objective clinical outcomes to monitor an individual’s level of function. He previously led the development of novel approaches to determine movements and loads experienced by a patient’s spine during high-velocity low-amplitude spinal manipulation.

The Redeemers

From humble beginnings—singing in the choir stalls at Church of the Redeemer in downtown Toronto—to street corners and backroom dives, the Redeemers revive the joy of doo wop close harmony singing. From the golden days of bluesy pre-rock’n’roll R & B to present day pop and top 40, this close harmony vocal group celebrates the sounds pioneered by classic groups like the Orioles, the Drifters, and the Flamingos, honed by The Beatles and Stevie Wonder, and re-imagined by more contemporary acts like the Backstreet Boys, Gnarls Barkley and Pharrell.
Bruce Scavuzzo - "Mr. Scavuzzo" has been singing in choirs since 8 years old, playing guitar since age 12, bass since age 16, and breaking hearts since birth. His incredible versatility in genres from funk rock, soul, R & B, fusion, folk, and classic rock have made him an in-demand collaborator around Toronto. He met the rest of the Redeemers while also singing in the choir at Church of the Redeemer in 2010. Bruce’s main claim to fame is he NEVER forgot to come to a rehearsal….at his place…ever…..
William Reid - Will is a Toronto-based musician, conductor, educator, and solo hog. When not singing with The Redeemers, Will lends his voice to a plethora of groups including The Elmer Iseler Singers, The Nathaniel Dett Chorale, Yasgurs Farm, and is the George Black Fellow at Church of the Redeemer. Will loves doing all the solos all the time and coming up with alternate names for the group, like the “Willtones” and “Earth, Will, and Fire.”
Tamara Mitchell – Tamara “Tammy” is a next level musical wunderkind who grew up singing and playing the violin and piano, often simultaneously. She currently holds court as a soprano lead at Church of the Redeemer in Toronto and is a highly decorated French Immersion teacher for the Toronto District School Board. Unmatched musical skills aside, Tammy actually remembers what we decided to do at the last rehearsal and is adept at pawning off teas she longer wants.
John Cowling - John has been singing professionally since he was in the single digits, and from an early age dedicated much of his time and passion as a musician and organizer to numerous charitable causes. He is an alto lead at the Church of the Redeemer. By day, John works in Advancement at the University of Toronto and by night, serves as Senior Executive Assistant to Mr. Reid. He has no other discernible talents and remains on permanent probation with the group.

Dr. Draw

There are very few names that can promise you won’t leave their gravitational pull without undergoing some kind of transformative emotional experience, and one of those names is Dr. Draw. The Moscow born electric violinist moved to Canada with his family in 1992, and learned his craft busking on the streets of Toronto where he earned his stage moniker as a healer of the spirit. Impossible to ignore, Dr. Draw was often solely responsible for bringing the people he encountered out of their somber mood and into a sunny one.
Being an integral part of the Toronto cultural scene for the last 23 years, Dr. Draw is known as the first violinist to venture from rock and classical into electronic music culture. and a Canadian heartthrob in the early 2000’s, though with ambitions and audiences reaching far beyond the borders of the city of his youth, Dr. Draw has seen much renewed success in the past 4 years. Internationally, he has performed for the opening of the Westfield World Trade Center 9/11 memorial in 2016, at the Canadian Embassy in Washington DC representing Toronto’s culture and diversity in 2017, alongside members of the Gypsy Kings in Mexico, for TV appearances in Sri Lanka, and was once again invited to perform at the Sovereign Art Foundation’s Art Fund in Hong Kong in 2019. Dr. Draw has also opened for many star acts these past 4 years including John Legend, Mariah Carey, Shaggy, 2 Cellos, and Drake.
However, Dr. Draw has always been more than a performing artist, he is just as well known for his productions and compositions, often teaming up with other uniquely talented musicians to bring out that specially crafted sound. Recently, he has had the pleasure to return to composing for dance after being chosen to create a song for Canada’s National Ballet School’s country wide initiative, Sharing Dance. Dr. Draw has also assisted in the scoring of several independent films, and more impressively, for the BBC documentary, ‘Death on the I-95’ concerning the opioid crisis in the USA.
With six albums attributed to his own stage moniker, and another released back in 2017 as part of the collaborative project Tokka consisting of himself and long-time musical partner and producer Acote, 2020 expects to see the release of two more albums associated with Dr. Draw including a seventh under his own moniker, and a debut album for project Nemesis. The explosive partnership between Dr. Draw and Canadian Beatboxing Champion Scott Jackson resulted in the creation of project Nemesis, a name based on that of the theoretical dwarf star thought to be a companion to our sun. Their sound ranges anywhere from Middle-Eastern influenced folk-music, to old school hip hop, to Mongolian chant, with a goal to continue a tradition of inventiveness in music, and build upon the inherent relationship between violin and the voice. Their collaboration in performance was kicked off of in 2017 on parliament hill for the Canada 150 celebrations, and since then they have experienced great success together onstage most notably performing at the Montreal Grand Prix, and for the Toronto Raptors’ infamous 2019, game 7 buzzer beater’s half-time show.
It is rare to find a musician nowadays who able to base his whole career entirely off of his creative work, but this is an accomplishment that can be attributed to Dr. Draw. His key to success cannot be explained beyond that his style speaks to and for his place in time. Dr. Draw embodies the enigma of improvisation, inspiring insatiable curiosity and awe wherever he goes
