SCA Secondary School Dry Grad & Formal Event Parent & Student Consent & Liability Waiver

EVENT DATE: Friday, June 2, 6:30 pm - Saturday, June 3, 2:00 am

LOCATION: EVOLVE Church, 1851 94 St NW, Edmonton

TRANSPORTATION: Busing to and from SCA Parking lot ONLY.  Buses leave at 6:30 pm   (NO Student is permitted to drive themselves to or from the Grad event location) If special circumstances need an alternate, pre-arranged pick up by a parent, please email the Grad Parent Committee at scagrad2023parents@gmail.com.

ACTIVITIES: Food (Formal Sit-Down Dinner, desserts, snacks, Midnight Taco Bar), Student-led Program at dinner, Music, Dancing, Hot Tubs, Inflatables, Outdoor & Indoor Games, Sports, Photographers, Fire Pits and more.

The SCA Secondary Dry Grad & Formal event is for SCA Secondary Students (only) graduating in June 2023 and who wish to celebrate with their fellow graduates in a safe and fun environment. Numerous hours of hard work, fundraising and several generous sponsors and donations have made this event possible. Thank you for all your support.

LIABILITY WAIVER

We acknowledge the event can have potential danger and that each individual participating is doing so at his/her own risk and in full knowledge of the hazards and potential hazards which are inherent in this type of activity.


In consideration of being allowed to participate in this activity and understanding that this event is organized by a parent group completely independent of Strathcona Christian Academy (SCA), we hereby assume all risk, and we hereby release and absolve SCA, their instructors, sponsors, and employees, whether on salary or representing SCA, and any members of the parent planning committee, the hosting property, Evolve Church, from all responsibility, liability or claims of any nature and kind which the student may have arising from any cause. I/We further agree to bear all costs of rescue and medical treatment rendered to the student arising from the activity.


Grads will arrive at or be dropped off at SCA by Parents or otherwise at 6:30 pm June 2, 2023, and picked up or leave from SCA when either first bus arrives (departure at 12:00 a.m.) or the final bus arrives after the last departure from Evolve at 2 am June 3, 2023. As noted, I/We agree to make alternative arrangements before June 3, 2023 if an alternate pick-up time is required. There are no other means of entering or leaving the event. There will be no overnight accommodation provided.  


Bag checks will be conducted before getting on the buses. I understand that my entrance into DRY GRAD is not guaranteed if I am found in possession of or under the influence of alcohol or drugs, and my parent/guardian will be contacted to pick me up.


Parent Volunteers will be chaperoning the event and will be present throughout the event, up to the drop-off at SCA at the end of the evening.


This is a DRY GRAD event - no tobacco, alcohol, vapes, marijuana or any controlled substances are permitted of any kind. A violation of this requirement will result in the individual being asked to immediately leave the event (Parents will be called for immediate pick up).


Evolve Church has been donated to use, free of charge, and NO activities or behavior considered to be disruptive to others or property will be permitted.


Photographs and/or videos may be taken by participants and posted on personal social media at this event. There will be photographers present, and photos will be posted to a private group Google site for attendees' access only.


Completion & signature of this agreement and waiver must be done and returned to the Grad Parent Committee prior to the event, or the Student will not be able to attend the event.

AGREEMENT STATEMENT STUDENT

I, the undersigned, understand that this event is drug (including marijuana) and alcohol-free, and no drinking of alcohol, smoking of any kind or vaping is permitted during the event, on the bus or at Evolve Church or at or near it's property. If I arrive or become under the influence, of any kind, while at the Dry Grad event, my parent/guardian will be notified and is expected to come to pick me up immediately, and I will not be permitted to return.

AGREEMENT STATEMENT PARENT/GUARDIAN

I, the undersigned, understand and agree to the conditions and rules listed in this form in its entirety. I understand that I am responsible for the transportation of my Grad to and from SCA (and can choose to allow them to drive themselves to and from SCA). I have discussed the above expectations, and my child understands they must abide by the outlined expectations set out by this statement, and refusing to comply may result in their being asked to leave the event.

WAIVER STATEMENT:

Furthermore, I do hereby waive, release, absolve, indemnify and agree to hold harmless the volunteers of the SCA Grad 2023 Parent Volunteers Committee and the property owner of Evolve Church from all claims/liability/assumed risks resulting directly or indirectly from or in connection with this event. I am signing this consent/waiver on behalf of my child and myself.
