How to get your COVID-19 vaccine certificate

Enter some information to learn how to get your enhanced vaccine certificate (with an official QR code) if you are an Ontario resident.

You can get your certificate no matter where you were vaccinated (for example, at a pharmacy, mass immunization clinic, hospital, or primary care setting) or if you have an active exemption.

How many doses of the COVID-19 vaccine do you currently have? (required)

You must select whether you received all your doses in Ontario.

Did you register the doses you got outside Ontario with a public health unit? (required)

You must select whether you registered the doses you got outside Ontario with a public health unit.

Select which health card you have (required)

You must select a health card type.

Do you identify as First Nations, Inuit, or Métis? (required)

If you are a non-Indigenous partner or household member of someone in this group, select "Yes."

Did you already contact your public health unit and get a COVID ID? (required)

Your public health unit gives you a COVID ID to use when you need future assistance with the Provincial Vaccine Contact Centre.

If you do not have one or live in a recently built neighbourhood, use the postal code of a shelter, library, or other community space in your area.

Get your enhanced vaccine certificate online through the provincial website.

You will need:

  • a green photo health (OHIP) card (you will need numbers on both sides of the card, expired cards will be accepted)
  • a modern web browser (the website is not compatible with Internet Explorer 11 or earlier)

To print a copy of your certificate, you can:

Call the Provincial Vaccine Contact Centre to get your certificate in the mail at 1‑833‑943‑3900 (TTY for people who are deaf, hearing-impaired or speech-impaired: 1‑866‑797‑0007), available in more than 300 languages, Monday to Friday (excluding holidays) from 8:30 a.m. to 5 p.m. Eastern time.

Get your enhanced vaccine certificate by calling the Provincial Vaccine Contact Centre.

You can get your certificate online or in the mail (if you do not have a printer or mobile phone).

If your information is not in the provincial database but you need another copy of your certificate, contact the host organization of the clinic where you got your doses or your public health unit. You need to register the doses you got outside Ontario with your public health unit.
