Information Session for Immunization of School Pupils Act Exemption

The following telephone appointments are for parents/guardians that are required to complete an information session when submitting a “Statement of Conscience and Religious Belief – Immunization of School Pupils Act” form for a child attending school. This information session involves first watching the Ontario Ministry of Health’s “Vaccine Education Video” and then scheduling a telephone appointment below to speak with an Ottawa Public Health nurse.
