How to obtain your electronic proof of vaccination

Download the QR code on the self-service portal

The self-service portal allows you to download your QR code, which is your electronic proof of vaccination.

General notice

Service in English is reserved for individuals covered by the exceptions stipulated in the Charter of the French language. If you have navigated to this content, you confirm in good faith that you are such an individual.

Once you have accessed the portal, you will be asked to provide identifying information, such as:

  • First name and last name
  • Date of birth
  • Health insurance number
  • Date of your first COVID-19 vaccination
  • Brand name of the first COVID-19 vaccine you received

Young people aged 14 years and under can also obtain their proof of vaccination by inserting their postal code into the self-service portal.

To confirm your identity and protect your personal information, you will be asked to provide the e-mail address or mobile telephone number provided at the time of vaccination to receive the download link.

General notice

If you plan to travel abroad and have received a dose of Moderna's bivalent vaccine, it is recommended that you download again your electronic proof of vaccination.

Download the QR code from the e-mail or text message

A few days after your vaccination, you’ll automatically receive an email or text so you can download your electronic proof of vaccination.

You will receive it, if you provided a valid e-mail address or a valid cellphone number and your health insurance number when you made an online appointment.

Instructions for downloading it will be provided in the message.

Even if a digital proof is available, you are strongly advised to keep the paper copy of your proof of vaccination that you received after your vaccination.

If you have difficulty getting your QR code

I have not received the e-mail or text message after receiving my vaccine doses
If have you not received the e-mail or text message after your doses, please visit the self-service portal This hyperlink will open in a new window.. You should also check your junk mail folder.

I don’t have an e-mail address or cell phone.
If you do not have an e-mail address or cell phone, call the line 1-877-644-4545 for the support you need.

An error message appears in the self-service portal
If an error message appears, you will not be able to download your proof of vaccination. This may occur if:

  • some of the information does not match the information in the Québec Vaccination Registry or the RAMQ register;
  • your parents are not registered with the RAMQ.

You must call 1-877-644-4545 to obtain the necessary support.

My proof of vaccination contains incorrect information
If the paper or digital copy of your proof of vaccination contains incorrect information, you must call 1-877-644-4545 to validate and rectify your information.

I got vaccinated abroad
Québec residents vaccinated against COVID-19 abroad can sign up at the Québec Vaccination Registry and obtain their proof of vaccination. For more information, see the Vaccinated for COVID-19 outside Québec: using the Vaccination Registry page.
