Toronto Grace Health Centre Strategic Planning Survey

The Toronto Grace Health Centre (Toronto Grace) provides medically complex and specialized care and services to individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation and Palliative Care.

As Toronto Grace nears the end of our current strategic period, we have partnered with Optimus SBR, an independent management consulting firm, to support the development of a new 5-Year Strategic Plan (2024-2029) and engage with internal team members, volunteers, patients, families, members of the public, and health service providers. The purpose of this survey is to gather perspectives to contribute to the development of the next Strategic Plan.

The survey will take approximately 10-15 minutes to complete. You may skip a question if you do not wish to answer it. Please do not press your browser’s “back” button while completing this survey, as this will cause it to close. Instead, there is a “Prev” button at the bottom of the survey that will take you to the previous page.

All responses will be kept anonymous, and we will not identify individuals. Please do not include any confidential information in any of your comments.
